Is there a "standard guideline" for written request to court for change date of scheduled appearance?

I am scheduled to appear in court on the same day I am also scheduled for surgery. In order for the court to consider rescheduling my appearance date, I am required to submit a written request. I would like to know if there is a specific guideline or "template" to follow and what details need to be stated in my official request?
